Whenever a doctor relocates offices he is removed from all insurance plans and has to reapply. This is a normal step in the process for relocating offices. Dr. Shaw has reapplied to all previous insurance plans. If you have received a letter stating that Dr. Shaw has been removed from your insurance no action on your part is required at this time.
